Some educational institutions and employment agencies request that applicants retrieve a letter of good standing from the College regarding their disciplinary and student conduct history. This may be requested when applying for admission to a college or university, a new job, or a permission form to attend an event.

 When requested by another educational institution, these forms are often referred to as: 

  • Common Application- Transfer College Report
  • Transfer College Report
  • Dean of Students Certification and Recommendation Form
  • College Official’s Report
  • Transfer Verification Form
  • Recommendation Form for Transfer Applicants

To initiate the process, please follow these steps:

  1. Learn the transfer process at the College/University/Institution that you are transferring to, acquire the documentation, and fill out the sections applicable to you.
  2. You must complete this online intake form and upload any documentation or provide appropriate weblinks, delivery email addresses, or other instructions given to you by the receiving institution. You may note in the “Details” section if there are specific deadlines.
  3. Should you be unable to upload documentation with the online form, you may submit the appropriate completed documentation and intake form to the Director of Community Standards in any one of these options:
    • In-person to Lynn campus- LW 171
    • In-person to Danvers campus- DB 132

Please be mindful that this process may take 3-14 days to complete, depending on the nature of the requested information and the volume of requests in our office. 

Please provide us with any specific mailing information, should you require the documentation to be sent out in any other way besides an email attachment. 

The transfer admissions website for each college you are interested in transferring to should tell you which application to use. These are some examples of applications you may see during your transfer process:

  • If you participated in a Massachusetts State Transfer eligible program, you will complete the MassTransfer Application
  • Many private colleges use the Common Application.
  • Some Colleges will have their application.
  • Some Colleges utilize electronic applications and forms, you should still complete the intake form to notify us that you wish for us to complete your form.

Visit the transfer admissions website for each College or call the admissions department at those Colleges for application instructions, required documents, and application deadlines.

Individuals with an active NSCC account: If you wish to check the status of your request, you may log in here, click “Associated Cases” at the top of the page, and find your submission to view the status. If it is marked as “Completed - Sent,” you should contact the receiving agency or institution with further questions. Otherwise, you may email communitystandards@northshore.edu with any questions.
